Honestly, there's a way that it could be framed as illegal, but nobody's managed to get it in front of the right judge.

It is not legal to practice medicine without a license.

If that was enforced on insurance companies they would have to get licensed medical professionals to review cases. It might not completely stop the problems because some doctors are just as bad, but surely it would help.
